In addition to keeping the wire length between the output any your DUT as short as possible, there are three things to check, that are typically overlooked:
1) The forward voltage drop in the ±output leads or ±sensing may be exceeding the specifications due to insufficient wire gauge.
2) The ±output leads MUST be a twisted pair (one twist/turn per inch, minimum)
3) The ±sensing leads MUST be a twisted pair (2-3 twist/turn per inch, minimum).
